Proof of Identity
- Supporting documentation
- Identification of the adverse information you wish to block
Supporting documentation can be in the form of, but not required to be, a Victim Determination Statement issued by:
A federal, state, or tribal government entity;
A non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination; or
A self-attestation document that you were a victim of trafficking that is signed or certified by a federal, state, or tribal government entity, court of competent jurisdiction, or an authorized representative of these entities.
